    1. Create a blank 300*100 pixel image.

    2. Fill the area with black.

    3. Select Filter --> Render --> Lens Flare and enter the following settings:

    4. Press okay. Create 2 more lens flares. (Try to spread them out.)

    5. Select Filter --> Sketch --> Chrome and enter the following settings:

    6. Press okay. Duplicate the layer by right-clicking and selecting "Duplicate Layer."

    7. Duplicate the layer 4 additional times.

    8. In the layers window, set the blending mode (upper-left box) to Lighten.

    9. Select Filter --> Distort --> Wave and enter the following settings:

    10. Press okay. Select the next layer down. Again, set the blending mode to Lighten. Apply a wave to this layer with the following settings:

    11. Select the next layer down. Set the blending mode to lighten. Apply a wave with the following settings:

    12. Select the next layer down. Set the blending mode to Lighten. Apply a wave with the following settings:

    13. Select the next layer down. Set the blending mode to Lighten. Select Filter --> Distort --> Twirl and enter the following settings:

    14. Press okay. Select the next layer down. This should be your last layer. Apply another twirl with the following settings:

    15. Select Layer --> Merge Visible or press Shift+Ctrl+E to merge all the layers.

    16. Select Image --> Adjustments --> Hue/Saturation or press Control+U. The following window should appear:

    17. Check Colorize. Drag the bars to get the color how you want it. Press okay. Your image should look something like this (with different coloring):

    18. Add a 3 pxl black-white-black border so it will look nice on any background. Here's my final product:

Now just put your name on it and you're done!

    Hope this was helpful. Regards,
