The coloring of the lips. Not sure, but maybe it should be more detailed and fit on the lips better, or maybe you were going for that look.
That smudging does a good job of starting to add depth to it by coming in front of the render. The smudging behind the render should be blurred...
Some of the effects are pretty sloppy and look kind of thrown together. But the c4ds and the overall result look nice.
