I quite like it. The background might be repetitive, but it works. I would like to see some more effects around his shoulder though, possibly adding some more flow to the piece.

Yes it doesn't really look finished. You should blend the render and add some effects.

 

Like the skulls but everything else doesn't seem to blend in well.
