This happens once in a ble moon for me. The "Additionally, a 404 Not Found error was encountered while trying to use an ErrorDocument to handle the request." part isn't anything to worry about, just saying it can't find the code 500 error document. The error is proberbly to do with IPB.
He's right, if you keep refreshing the page you can get 500's, 404's, 403's etc. Especially when there is the mySQL error: too many connections. Just refresh once in a minute or something.
