It was deliberate. They say on the special features on Episode II as Jango hits his head on the hull door of Slave I because he can't see panoramically through his helmet so he hits his head on things alot, this trait was cloned into the Republic Troops whom later became Imperial Stormtroopers.
There is also another blooper in LOTR I caught. In the Two Towers when Frodo and Sam get to that ruined type city. It shows frodo on the ground, the chain of his necklace is wrapped aroun the button on his shirt. When the camera goes back to him it's back to normal.
