No. In most cases, if you email them or fill out a form on their site (only on some), then they will add you. I know that Google sends a spider-bot to your site to help organize it into catergories for searches.
Uh yea you do have to pay... It's not much, but you still have to pay Just check the sites before you actually ask =/
Oh, I didnt know that. The last time I was subscribed to a search engine, it was free (Google). Or it may have been my host.

Spiders/Crawlers could pick up your site auto, but there's only a very small chance it does. Both google and yahoo are free to add your site, but if you're not the popular type, or don't have meta tags and all that other stuff, you'll just end up in the bottom
I remember getting something in the mail from google saying if i paid so much a year my site would be on the search engine faster then what it is now. Meh i didn't do it. Waste of money if it's not a big offical business site.
